The ISX11.9 is Cummins' answer to the US EPA 2010 emissions standard in the 12-litre class: in 2010 it replaced the 10.8-litre ISM, which had run in distribution trucks, tippers and buses since the 1990s, and adopted the architecture of the big ISX15 at a reduced scale – overhead camshaft, XPI common-rail injection, cooled EGR, particulate filter and SCR with urea. As early as 2013 it was renamed ISX12; the technology stayed, the name followed the marketing of the X programme.

The number in the name is the output in hp; suffixes such as ST, EV, R, MC or AS mark torque curves and calibrations for particular transmissions and duties. The ISX11.9 marks the transition to SCR technology in this class: for the first time it needed urea, but in return cut fuel consumption compared with the EGR-only EPA 2007 generation, and it was fitted mainly in Peterbilt, Kenworth and Freightliner distribution vehicles and in coaches.

On used engines the early SCR systems are the checkpoint: NOx sensors, dosing module and particulate filter with regeneration history; the base engine is regarded as durable if the EGR was kept clean.
